To improve the process of capturing all required information when obtaining consent for treatment of minors at special events. It was determined that the form used to record the consent had an unacceptable rate of missing information. The team redesigned the form, gave training to the EMS staff in its use and is currently collecting data on the number of forms with missing information.

It is the goal of the team to have 90% of all forms correctly completed with no missing information.
Data from 1-2007 through 11-2007 found 1 of 32 forms had missing information. The team plans to continue data collection through the summer 2008 when the largest groups of minors are on campus for summer enrichment programs and camps.
